Eczema drugs tacrolimus (Protopic) and pimecrolimus (Elidel): cancer concerns.

چکیده
© 2005 CMA Media Inc. or its licensors Reason for posting: Many patients with eczema, or atopic dermatitis, are prescribed the topical immunomodulators tacrolimus and pimecrolimus. The drugs are often given to people for whom the potential side effects of topical corticosteroids (e.g., systemic absorption, skin thinning, telangiectasia) are a concern. However, the US Food and Drug Administration (FDA) recently reviewed the safety of these agents and warned that they may be associated with a risk of cancer.

An Appropriate Response to the Black-Box Warning: Corrective, Barrier Repair Therapy in Atopic Dermatitis.
Due to years of sophisticated research on T cell function, many dermatologists have viewed atopic dermatitis (AD) largely as an inflammatory disorder of TH1/TH2 imbalance. Hence, therapy has largely consisted of topical immunomodulators and/or steroids. The imposition of "black box" warnings about the potential toxicity associated with prolonged use of the immunosuppressive drugs, tacrolimus 0....
متن کاملImmunomodulation and safety of topical calcineurin inhibitors for the treatment of atopic dermatitis.
Atopic dermatitis (AD) is a chronic or chronically relapsing inflammatory skin condition that primarily affects children. Topical corticosteroids have been the mainstay of treatment since the late 1950s. While providing excellent short-term efficacy, topical corticosteroid usage is limited by potential adverse effects, including impairment of the function and viability of Langerhans cells/dendr...
متن کاملPimecrolimus in dermatology: atopic dermatitis and beyond.
Pimecrolimus is a calcineurin inhibitor developed for the topical therapy of inflammatory skin diseases, particularly atopic dermatitis (AD). Pimecrolimus selectively targets T cells and mast cells. Pimecrolimus inhibits T-cell proliferation, as well as production and release of interleukin-2 (IL-2), IL-4, interferon-gamma and tumour necrosis factor-alpha. Moreover, pimecrolimus inhibits mast c...
متن کاملTopical calcineurin inhibitors in dermatology. Part I: Properties, method and effectiveness of drug use
Topical calcineurin inhibitors (TCI) are a relatively new class of drugs used in dermatology. There are two drug forms available - tacrolimus 0.03% or 0.1% ointment and 1.0% pimecrolimus cream. The drugs act by inhibiting synthesis of proinflammatory cytokines. The only approved indication for using TCI is treatment of atopic dermatitis. The TCI may be used as an alternative therapy to corticos...
متن کاملSafety and efficacy of tacrolimus ointment versus pimecrolimus cream in the treatment of patients with atopic dermatitis previously treated with corticosteroids.
Adult and pediatric patients (n = 347) with atopic dermatitis enrolled in three multicenter, randomized, 6-week studies who had previously used steroids were analyzed to examine the null hypothesis that improvement in atopic dermatitis initiated after prior treatment with steroids eliminates any subsequent treatment differences between tacrolimus ointment and pimecrolimus cream. Of these patien...
